How to Report a Retiree's Death
To report a Retirees Death to SHBP, please complete the form further below. You should also contact the State Retirement System (e.g., ERS or TRS).
After You Report the Retiree’s Death
- You will receive an acknowledgement email
- SHBP will transition any covered Surviving Spouse and/or Surviving Child eligible to continue coverage to a separate SHBP account so he/she may access their coverage information.
- If a refund is owed for premiums submitted to SHBP after the Retiree’s month of death, SHBP will contact the Personal Representative within 3 business days or mail a letter to the Retiree’s address on file within 7 days requesting the Estate complete an IRS W-9 Form.
