Contractor Access Agreement

SHBP Employers should use this form to request or remove access to the SHBP Enrollment Portal and/or SHBP State Health Repository Tool (SHRT) for their contracted vendors, including but not limited to state agencies acting as contracted vendors for human resources, financial, and/or payroll related services.
