SHBP must have valid banking information on file for every SHBP Employer. If you are a new Employer or an existing Employer who needs to update their payment method, please select the applicable icon below to complete and sign the necessary form. Please note existing Employers should continue to submit their payments as normal until SHBP approves a change to their payment method.
